The Family Nurse Practitioner (FNP) will provide high-quality care to geriatric patients in skilled nursing facilities. This role is responsible for patient admissions, follow-up care, discharge planning, and ongoing management of rehab and long-term care patients. The FNP will work closely with supervising physicians, facility leadership, nursing staff, patients, and families to support quality patient outcomes in acute and post-acute care settings. Essential Functions
Provide full-time clinical coverage by rounding at assigned skilled nursing facilities.
Evaluate and treat rehab, long-term care, and custodial nursing facility patients.
Complete physical assessments, document findings, and develop treatment plans.
Order, administer, review, and interpret diagnostic tests as clinically appropriate.
Prescribe medications and treatments in accordance with state regulations and company policies.
Monitor patient conditions, identify changes in health status, and adjust treatment plans as needed.
Collaborate with physicians, nursing staff, and interdisciplinary care teams to coordinate patient care.
Communicate care plans and updates to patients, families, and healthcare teams.
Complete clinical documentation, billing, and regulatory requirements in a timely and accurate manner.
Maintain professionalism and represent CIMPAR with courtesy and discretion.
Schedule
Monday to Friday rounding at assigned skilled nursing facilities.
On-call coverage every third weekend.
