Positon title: Mental Health Professional – Targeted Re-Entry Services Jail Services

Non-Exempt Position

Responsible to: Program Director

Major duties

Under the supervision of the Program Director and Program Supervisor, the Mental Health Professional delivers community-based behavioral health and care coordination services to participants. The position requires independent decision-making, effective care coordination, regulatory compliance, and collaboration with multidisciplinary teams to support participant recovery and successful community integration.

• Clinical Assessment and Care Planning
• Conduct comprehensive biopsychosocial, behavioral health, and Health-Related Social Needs (HRSN) assessments consistent with Medicaid Re-Entry Demonstration requirements, agency standards, and applicable Washington State regulations.
• Utilize clinical judgment to identify participant strengths, barriers, risks, and service needs to support successful community re-entry.
• Develop, implement, and monitor individualized Re-Entry Care Plans that are person-centered, trauma-informed, culturally responsive, recovery-oriented, and strength-based.
• Regularly evaluate participant progress and modify care plans as clinically indicated.

• Care Coordination and Service Delivery
• Provide targeted case management and behavioral health support services under Washington State's Medicaid Re-Entry Demonstration (1115 Waiver) and applicable managed care requirements.
• Coordinate transitions of care between correctional facilities, behavioral health providers, medical providers, housing resources, employment services, and community-based organizations.
• Facilitate participant access to behavioral health treatment, substance use treatment, medical care, benefits, transportation, housing, and other social support services.
• Demonstrate effective crisis identification, intervention, de-escalation, and referral practices consistent with agency protocols and participant safety needs.

• Interdisciplinary Collaboration
• Function as an active member of a multidisciplinary treatment team by contributing clinical observations, recommendations, and service updates.
• Collaborate effectively with correctional staff, healthcare providers, probation officers, courts, managed care organizations, community partners, and other stakeholders to support continuity of care.
• Participate in case consultations, staffing meetings, utilization reviews, and treatment planning discussions.

• Documentation and Regulatory Compliance
• Maintain timely, accurate, and complete clinical documentation in accordance with:
• WAC 246-341
• RCW 71.05
• RCW 71.24
• Medicaid Re-Entry Demonstration requirements
• HIPAA and 42 CFR Part 2 confidentiality requirements
• Agency policies and procedures
• Demonstrate accountability for meeting productivity, documentation, billing, and reporting requirements.
• Apply knowledge of applicable federal, state, and local regulations to ensure compliance in all aspects of service delivery.

• Community Engagement and Resource Development
• Develop and maintain effective working relationships with community agencies, service providers, and referral partners to expand participant access to resources and support services.
• Identify gaps in community services and advocate for participants to obtain needed resources.
• Represent the organization professionally at community meetings, care coordination events, and collaborative initiatives as assigned.

• Professional Accountability
• Demonstrate ethical decision-making, professional boundaries, and adherence to the National Association of Social Workers (NASW), American Counseling Association (ACA), or other applicable professional standards.
• Participate in supervision, training, performance improvement initiatives, and continuing education activities.
• Support agency quality improvement, CARF accreditation activities, and organizational performance outcomes.
• Promote a culture of recovery, inclusion, participant-centered care, and continuous quality improvement.

Salary

$29.94 - 46.28/hr DOE
