We are seeking dedicated Advanced Practice Providers (Nurse Practitioners and Physician Assistants) to join our innovative, value-based healthcare model. You will deliver in-home clinical services to patients with complex needs, all while collaborating with a team of experts to improve patient outcomes and enhance quality of life.

Base Salary/ Wage Range $135,000 to $145,000 plus annual bonus.
Compensation for the role is commensurate with the candidate’s qualifications, skills, competencies, and experience and may fall outside of the range shown.
