Licensed Practical Nurse (LPN) – Primary Care Overview

Aptive Resources is hiring Licensed Practical Nurses to support direct patient care delivery within the Primary Care PACT team at the Dayton VA Medical Center.

Primary

Responsibilities

Obtain patient histories and assess nursing care needs Assist with medication administration and document in EHR Perform skin assessments and preventive care interventions Carry out physician orders and document in patient medical record Support emergency response and initiate life-saving measures as needed Participate in team huddles and quality improvement meetings

Minimum Qualifications

Graduate of an ACEN- or CCNE-accredited LPN program Current, active Ohio LPN license Current BLS certification; ACLS required if assigned to specialty clinic Minimum 1 year direct patient care within the last 3 years; VA experience preferred Must pass background investigation (Tier 1/Low Risk); PIV card required Must complete required VA mandatory training prior to start
