Hospice Savannah Inc. - SAVANNAH, GA 31406Job Summary The Scheduler is responsible for coordinating and scheduling outside medical appointments for PACE participants in accordance with participant care plans and IDT direction, while maintaining accurate medical appointment records and performing other duties as assigned by PACE Center management. QualificationsMinimum Qualifications Before working independently, meet a standardized set of competencies for the Scheduler position established by the PACE plan (CMS required). Before engaging in participant care, be medically cleared for communicable diseases and have up-to-date immunizations and vaccines (CMS required). Have not been convicted of criminal offenses involving Medicaid, Medicare, other health insurance or health care programs, or social services programs under title XX of the Act (CMS required). Have not been excluded from participating in Medicare and Medicaid programs (CMS required). Have not been convicted of criminal offenses pertaining to physical, sexual, drug, or alcohol abuse (CMS required). Competencies, Preferred Knowledge, Skills, and Abilities High school diploma or equivalent (Preferred). Experience in medical scheduling, medical records management, or healthcare administrative support (Preferred). Experience working in Electronic Health Records (EHRs) (Preferred). Experience coordinating medical appointments with hospitals, specialty providers, or outpatient clinics (Preferred). One year of experience working with a frail or elderly population or in a healthcare setting serving older adults (CMS preferred). Physical Demands and Work Environment This position is primarily sedentary and is performed in an office environment. The Scheduler routinely sits for extended periods while working on a computer and communicating via telephone and email. The role requires frequent visual focus and manual dexterity to enter and review appointment data, medical records, and documentation, as well as the ability to communicate effectively in verbal and written formats. The work environment typically involves a moderate noise level typical of an office or healthcare setting. Occasional standing, walking, bending, or reaching may be required while performing duties within the PACE Center. The position may require the ability to lift or carry light office materials such as files or documents of up to 20 pounds. Position Type and Expected Hours of Work This position is classified as PRN, non‑exempt, generally scheduled to work as needed during the days of

Monday

Friday, 8am- 4:30pm. This position may require occasional variations in schedule, including additional hours, based on operational and organizational needs. Travel No travel required

Requirements

Candidates should ideally have a high school diploma and experience in medical scheduling or EHR systems. CMS-required clearances regarding criminal history and medical immunizations are mandatory.
