Assist residents with activities of daily living (ADLs) according to care plans and established procedures. Support residents with lifting, turning, positioning, transferring, and transportation. Coordinate dining services, including meal setup, delivery, feeding assistance, and documentation. Deliver nutritional supplements and document intake as required. Assist with resident weighing procedures and report significant changes. Monitor and report skin condition concerns, including pressure areas and skin tears. Assist with toileting programs and personal hygiene care. Complete required documentation and flow sheets accurately and timely. Communicate changes in resident condition to nursing staff and supervisors. Provide supportive services to nurses and healthcare staff as needed. Maintain a safe, respectful, and caring environment for residents. Must Have Active Certified Nursing Assistant (CNA) certification in good standing, OR completion of a state-approved nursing assistant training program with competency exam completed within 120 days of employment. Ability to communicate effectively with residents, families, and interdisciplinary healthcare teams. Ability to provide compassionate, resident-centered care. Ability to perform physical caregiving duties, including lifting, repositioning, and transferring residents. Nice to Have High School Diploma or GED. Current CPR/BLS Certification. Minimum one (1) year of CNA experience in a long-term care setting.
