About the role

The RN will conduct comprehensive patient assessments and deliver skilled nursing services, including wound care and IV therapy, within patients' homes. They are also responsible for coordinating care with physicians and providing emotional guidance and education to families.

We are looking for a skilled and compassionate Registered Nurse (RN) to join our Home Health & Hospice team. As a field RN, you will play a vital role in delivering high-quality, patient-centered care to individuals in their homes—promoting comfort, dignity, and independence.

Key Responsibilities

Conduct comprehensive patient assessments and develop individualized care plans Provide skilled nursing services including wound care, IV therapy, symptom management, and medication administration Monitor and document changes in patient condition and coordinate care with physicians and interdisciplinary team members Support patients and families with education, resources, and emotional guidance throughout the care process Participate in the on-call rotation, including evenings, weekends, and holidays as needed Ensure timely, accurate documentation in compliance with agency policies and regulatory standards

Qualifications

Current RN license in the State of Texas Current CPR certification (BLS required) Minimum of 1 year of clinical nursing experience; home health or hospice experience strongly preferred Bilingual (English/Spanish) preferred Strong clinical judgment, time management, and communication skills Reliable transportation, valid driver’s license, and automobile insurance (required for field-based visits)

Requirements

Candidates must hold a current Texas RN license, BLS certification, and have at least one year of clinical nursing experience. A valid driver's license, automobile insurance, and reliable transportation are required for field visits.
