As a Home Health RN, you'll oversee a caseload of patients within your assigned territory while partnering with physicians, therapists, and interdisciplinary team members to deliver exceptional care.
Complete comprehensive Start of Care, Resumption of Care, and Recertification assessments.
Develop individualized care plans that support patient goals and independence.
Coordinate care with physicians, therapists, social workers, and caregivers.
Educate patients and families on disease management, medications, and safety.
Complete timely documentation and OASIS assessments.
Monitor patient progress and adjust care plans as clinical needs change.
Supervise Home Health Aides and collaborate with LPNs as appropriate.
Participate in case conferences and interdisciplinary care planning.
Help improve patient outcomes while maintaining compliance with Medicare Conditions of Participation and agency quality standards.
