Baptist Health is currently looking for a Clinical Pharmacist to join our Pharmacy team at Baptist Medical Center located in Fernandina Beach, FL.
00 am - 3:30 pm, Monday through Friday and every fourth weekend.
This role provides clinical pharmacy coverage for Med/Surg Progressive Care, ICU, ED and OR units, collaborating closely with physicians, nurses, and interdisciplinary care teams to optimize medication therapy. On a daily basis, the pharmacist reviews medication orders, monitors patient responses, performs medication reconciliation, and provides evidence-based recommendations to improve patient outcomes. Throughout the week, they manage complex medication regimens for patients with acute and chronic conditions, assist with transitions of care, and support antimicrobial stewardship and medication safety initiatives. The pharmacist also serves as a medication expert and educational resource for healthcare providers while contributing to quality improvement efforts across the inpatient setting.

A strong background in clinical pharmacy experience and/or a completed PGY1 is preferred. Candidates must possess a Pharmacist license in the State of Florida in order to be hired into this position.
