About the role

The Physical Therapist provides skilled home health care services based on physician-led plans of care. Responsibilities include assessing patient functional status, directing treatments, and instructing families on therapeutic appliances.

Job Description

The physical therapist will provide physical therapy services to patients according to a written physician's plan of care which will, in part, include: Assessing and evaluating therapeutic/rehabilitative/functional status, and participation in the development of the total plan of care, as well as evaluating the home environment. Assessing for muscle strength, mobility, gait, ROM--potential for rehab. Directing physical therapy treatment. Instructing patients and families/caregivers in the use and care of therapeutic appliances. Determining priority needs for physical therapy. Reporting to physician patient's reaction to treatment or changes in condition.

Responsibilities

The Physical Therapist will be responsible for visiting patients in their homes and providing skilled home health care clinical visits, providing care for individuals based on personalized Plan of Care. Qualifications/Requirements:(preferred unless required by law) Current unrestricted PT license Minimum one (1) year of home care experience preferred. Three (3) to five (5) years of clinical experience and preferred previous supervisory experience. Clean background and criminal record CPR certified Knowledge of computerized documentation and office software required Preferred although not mandatory: experience with OASIS (although this can be trained)
