(Temporary Full Time) - CASE MANAGER (MSW)

Responsibilities

• Canyon Ridge is a 157-bed acute, locked psychiatric facility offering services to adolescents ages 13-17, adults and older adults.
• We believe in an individualized, patient-focused approach to diagnosis and treatment.
• For some, anxiety or drug dependence looks different from others with the same diagnoses, and in some cases, behavioral health and substance use disorders occur simultaneously.
• In these cases of dual diagnosis, we treat the patients in the best way possible given their specific needs.
• Responsible for providing quality case management, utilization review activities and psychotherapeutic services to psychiatric patients and their families, spouse, significant other; to serve as a member of interdisciplinary team supporting the organization’s treatment program and philosophy and assure the deliverance of quality treatment to psychiatric patients and their families.

Qualifications

• Masters degree in Social Work 2 years of experience in inpatient setting preferred; Written and oral communication skills, knowledge of utilization management & skills in case management, time management, discharge planning and crisis intervention common to acute psychotic as well as to non-violent crises intervention practice.
• Knowledge of acute adult psychological disorders Assessing clients' needs and developing individualized care plans.
• Coordinating with healthcare providers, social services, and other stakeholders.
• Monitoring and evaluating clients' progress and adjusting care plans as needed.
