AHN Primary Care Institute is seeking a Geriatric Medicine Physician to join our growing team in Erie, Pennsylvania. In this role, you will deliver high-quality, person-centered care in a consultative/co-management capacity to older adults with complex medical and social needs.
Work closely with primary care clinicians and specialty providers, where you will help guide care plans that promote independence, dignity, and quality of life for aging patients.
Practice in a supportive, collaborative environment with access to multidisciplinary team-based resources to ensure coordinated, comprehensive care for every patient.
Primary outpatient, with potential for limited practice in skilled nursing facilities/assisted living communities.
Clinical expertise in geriatric medicine, with a focus on delivering individualized, Age-Friendly care that enhances patient outcomes and quality of life
Exceptional communication and interpersonal skills, enabling effective collaboration with patients, families, caregivers, and multidisciplinary care teams
A commitment to lifelong learning and professional growth, with enthusiasm for contributing to academic activities such as mentoring trainees, participating in educational programs, and staying current with advances in geriatric care and evidence-based best practices
