Healthcare United is seeking a Board-Certified or Board-Eligible Hospice and Palliative Care Physician to join a community-based healthcare organization in Connecticut. This role includes a mix of inpatient, outpatient, and home-based palliative care along with hospital-based coverage. The position offers a collaborative, interdisciplinary environment with strong clinical support. Provide hospice and palliative care across inpatient, outpatient, and home-based settings Conduct goals-of-care discussions and advance care planning Manage pain and complex symptoms including dyspnea, nausea, fatigue, and delirium Participate in hospital coverage including admissions and daily rounding Collaborate with interdisciplinary teams including nursing, social work, and case management Support transitions of care between hospital, hospice, and community settings Participate in shared call coverage Maintain accurate and timely EMR documentation Engage in quality improvement and program development initiatives
Board Certified or Board Eligible in Hospice and Palliative Medicine, Internal Medicine, or Family Medicine Eligible for Connecticut medical licensure Active or eligible DEA registration Strong clinical and communication skills Ability to work effectively within a multidisciplinary team Commitment to patient-centered care
