PRIMARY FUNCTIONS Provide primary care to LVIM patients, including histories, exams, lab tests, prescriptions and referrals within prescribed up to date guidelines and protocols.; Obtains patient histories and performs physical examinations on adolescent, adult, psychiatric, gynecological, and geriatric patients; Orders appropriate laboratory and diagnostic procedures for the patients presenting problems; explaining necessity, preparation, nature, and anticipated effects to the patient; and interpret laboratory and diagnostic procedures common to primary care acuity patients such as but not limited to basic radiographic films (extremities, chest, and abdomen), urinalysis, and complete blood count (CBC); Perform accurate and timely documentation of histories, subjective findings, physical exams, plans, prescriptions and follow-up in Electronic Health Records System within 72 hours of the visit. Document patient chart entries in Subjective, Objective, Assessment & Plan (SOAP) format; following LVIM policies, and Medicaid and Medicare guidelines. Performs diagnostic procedures such as pelvic examinations with specimen collection, removal of impacted cerumen, rectal examination with hemoccult, removal of foreign bodies, and basic life support. Focus on patients with chronic, unstable, or multi-system problems but does include acute simple illness as well as healthcare maintenance. Provide members and patients information concerning their evaluation, diagnosis, treatment, and prognosis. Engage members in participation and decisions involving their healthcare. Provide counseling for patients with diet, hygiene, and preventative healthcare Actively participate and engage in quality improvement for clinic efficiencies and improving member quality of care; Consult supervising physician or colleagues when needed for complicated or case is outside the scope of practice. Advise the Medical Director/ Operations Director/COO of any situations that may adversely impact the efficiency of the medical department.
SECONDARY DUTIES Actively participates in the building of good rapport with staff and colleagues through attendance of provider staff meetings, communicating in a respectful and courteous manner to staff, and committing to teamwork consistent with the mission/vision of the Company; Attends and participates in medical staff and/or facility committees; Assists physicians, supervisors, company personnel, and/or health department personnel in collection or analysis of quality or dashboard data.
Educational and/or Professional Qualifications: Education: Master’s Degree in Nursing required Certification: National Certification in a specialty area from American Nurses Credentialing Center (credentialing arm of the American Nurses Association), the American Academy of Nurse Practitioners, or Pediatric Nursing Certification Board, and Basic Life Support (BLS) Licensure: Current Licensure or State endorsement (with prescribing privileges) as a Nurse Practitioner. Previous Experience: At least one (1) year of primary care experience as a Nurse Practitioner.
