Provide in-home physical therapy treatments and therapeutic exercises to help patients improve mobility and independence. Monitor patient progress and communicate updates to the supervising Physical Therapist.

National Home Care Agency is seeking a compassionate and reliable Physical Therapist Assistant (PTA) to provide in-home physical therapy services under the supervision of a licensed Physical Therapist. This role focuses on helping patients improve mobility, strength, and independence in the comfort of their own homes.

Responsibilities

Provide physical therapy treatments according to the established plan of care Assist patients with therapeutic exercises, gait training, and mobility activities Monitor and document patient progress Educate patients and caregivers on home exercise programs Communicate patient updates to the supervising Physical Therapist Complete timely and accurate documentation Travel to patient homes within assigned areas

Qualifications

Associate Degree from an accredited PTA program Active state PTA license CPR/BLS Certification Home health experience preferred Valid driver’s license and reliable transportation Strong communication and documentation skills
