About the role

The coordinator manages a caseload of families, providing support services and creating individualized support plans to ensure children remain outside of foster care. They facilitate support groups and coordinate with community partners to provide training and resources to caregivers.

Job summary

Responsible professional work delivering family support services utilizing a broad range of community services and natural support systems. Involves a broad range of training and development activities, assessment services, and consultations and requires quality documentation of services provided. Participates in organization’s performance and quality improvement efforts.

Essential functions

1.

Maintains a caseload of 12-15 families and not to exceed 18.

2.

Maintains weekly contact with all families on active caseload by means of home visits, office visits, phone contact or correspondence.

3.

Facilitates support group for relative caregivers regularly.

4.

Coordinates with community partners to provide information and training to caregivers.

5.

Ensures families who complete the program will accomplish Support Plan goals.

6.

Ensures that children living with relative caregivers will remain outside of foster care one-year post discharge from the program, in accordance with Kinship program expected outcomes.

7.

Creates, implements, and evaluates effectiveness of each family’s Support Plan.

8.

Completes Assessments, program statistics, and other required documents in the specified amount of time.

9.

Ensures that families have follow-up linkages into the community once completed program in accordance with Kinship program expected outcomes.

10.

Family Support Coordinator follows-up on all referrals to determine if family received services.

11.

Assists families navigate community resources and addresses barriers, including transportation, advocacy, legal, medical, and mental health needs.

12.

Meets with Program Manager on a weekly basis for supervision to review cases, training, and professional development and program development.

13.

Works within the philosophy and mission of the Children’s Home Network.

14.

Participates in quality assurance reviews of the program services, outcomes, and records.

15.

Works within the philosophy and mission of the Children’s Home Network.

16.

Performs other related duties as assigned.

Minimum qualification requirements

Education & Experience

Bachelor’s Degree from an accredited university or college in Human Services field, with one year of experience working with children and families.

Bi-lingual (Preferred)

Licenses & Certifications

Must possess and maintain a valid Florida driver’s license with no record of criminal driving offense of license suspension.

Must possess means of providing job related transportation and show proof of required insurance. Must be insurable under CHN’s current auto insurance policy.

Must be able to work flexible hours, including evenings and weekends.

Must successfully complete and maintain FDLE clearance, Federal background and State criminal background check and sexual predator screening.

Current CPR and First Aid Certification.

Competencies & professional development

Annual Training Requirements: The following training topics are required annually: Sexual

Harassment, Blood-born Pathogens, Workplace Accident Prevention and Reporting,

Confidentiality and Privacy Practices, Diversity/Cultural Competency, Child Abuse and Neglect

Mandated Reporter Statutes and Methods, Prevention of Violence in the Workplace, OSHA – related training, Crisis Prevention, and Program Goals.
