About the role

The Nurse Practitioner is responsible for daily patient rounding and the diagnosis and management of acute and chronic conditions. They will collaborate with interdisciplinary care teams and utilize AI-integrated EHR for documentation.

This role offers predictable schedules, no nights or on-call coverage, and strong clinical support—allowing you to focus on patient care without administrative overload. New graduates are welcome and supported through a structured mentorship program and will receive ongoing executive support.

Responsibilities

Daily patient rounding Diagnose and manage acute and chronic conditions Collaborate with interdisciplinary care teams and facility leadership Complete documented notes in EHR with integrated AI that allows quick seamless documentation

Qualifications

Active or eligible NP or PA-C license DEA and national certification (or ability to obtain) SNF experience preferred, not required. New Grads are welcome to apply.
