The RN Case Manager performs initial and ongoing physical assessments to develop and revise comprehensive care plans for patients and their families. They administer prescribed treatments and medications while coordinating discharge planning and providing health education.

Candidates must hold a valid state RN license, BLS/CPR certification, a valid driver's license, and auto insurance. A minimum of one year of recent acute care experience is required, with a degree in nursing and home health experience preferred.
