The Advanced Practice Registered Nurse (APRN) or Physician Assistant (PA) provides comprehensive primary and preventive medical care to geriatric patients in their home, nursing facilities and occasional outpatient clinic coverage in the Family Health Center as needed. Care is delivered in collaboration with CHMG physicians, with a focus on optimizing health, independence, and quality of life for older adults. This role requires regular travel to community-based care settings.

Requires a Master's degree in Advanced Practice Nursing or completion of a Physician Assistant program with an active New Hampshire license and DEA registration. A minimum of 2-3 years of clinical experience in geriatrics or primary care for older adults is required.
