Generosity guides us as we honor our legacy and continuously grow through a culture of curiosity, creativity, and optimism and as we advocate for people with disabilities to lead full and equitable lives.Job DescriptionAHRC NYC is seeking a Fee for Service Physical Therapist for two days a week at their Manhattan day program serving adults with developmental disabilities The Physical Therapist is responsible for, evaluating the patient’s level of physical function, working with the patient/family to provide training and/or assistive devices to eliminate, or reduce areas of deficit, which are usually associated with gross motor disorders and functioning.

The Physical Therapist also works with other professional staff to establish rehabilitation and maintenance goals and activities.Pay Rate: $45 for 30 minutes.Schedule: 8:30am - 3pm Wednesday and ThursdayDUTIES AND RESPONSIBILITIES:Evaluate the physical therapy needs of the patient and provide physical therapy services for the patient as ordered by the Physician/Nurse Practitioner.Perform clinical tests to assess functional limitations and the potential for restoration.

Develop a plan of care with restorative and/or maintenance goals to meet identified needs.Perform applied physical therapy procedures to achieve identified goals, (i.e., therapeutic exercises, functional training).Instruct patients/family in the use of assistive modalities and appropriate physical therapy procedures.Observe, record and report to the Physician/Nurse Practitioner the patient’s reaction to treatment and any changes in the patient’s condition.Work with the health team to shorten the patient’s recovery from illness and the management of residual disability.Initial evaluation will be completed at the first available appointment.Make physician contact, followed by a written report, to the Physician and/or professional staff to report initial findings, goals, treatment frequency and estimated length of service.Re-evaluate patients every 3 months to assess progress attained and completes a written progress report for the patient's record.Maintain communication with the physician, keeping in contact through written reports, attendance at meetings and telephone calls.Have ongoing communication with AHRC staff concerning information that will influence or change the provision of services, including any unusual situations observed during the course of service.Participate in interdisciplinary care planning for individual patient needs.Complete and submit all necessary documentation within 1 day of service.

Documentation includes type of treatment given, modality used and patient reaction.Fit assistive devices for self-care and ambulation; train patients in the use of devices and monitors for continued need and ability to use.Participate in in-service educational programs.Submit discharge summaries at the completion of services including, care provided, equipment ordered and any recommendations for follow up care.Participate in Quality Assurance activities, as requested.QualificationsCurrent licensure as a Physical Therapist in the State of New York.Graduate of a Physical Therapy Curriculum approved by the American Physical Therapy Association or the Council on Medical Education of the American Medical Association.A minimum of one year Physical Therapy experience in a Health Care setting.Demonstration of considerable knowledge of current standards, procedures and techniques of Physical Therapy.Additional InformationAHRC New York City is an Equal Opportunity Employer.

Requirements

Candidates must hold a current New York State Physical Therapy license and have graduated from an APTA or AMA approved curriculum. A minimum of one year of experience in a healthcare setting is required.
