Job Description

As a Care Manager, you will be a trusted guide for patients and families navigating complex health journeys. This role blends your clinical expertise with a holistic approach to care, addressing medical, behavioral, and social needs. Creating personalized care plans that go beyond diagnoses, addressing social determinants and emotional well-being. Qualifications

What you'll need for success: Active Illinois RN license Strong background in care management as well as complex and chronic condition management Skilled in motivational interviewing, patient education, and patient advocacy Ability to work independently in a remote environment while staying connected to the team Comfort with technology and multiple EMR systems SPECIALTY QUALIFICATIONS Minimum of five (5) years of clinical experience across diverse healthcare settings Behavioral Health: Requires advanced knowledge in psychiatric conditions, crisis intervention, and therapeutic communication and/or PREFERRED QUALIFICATIONS & CERTIFICATIONS CONSIDERED Compact License Bachelor’s Degree Master’s Degree Specialty Certification in Care Management (CCM) Psychiatric-Mental Health Nursing (RN-BC), for behavioral health-specific roles Multi-Lingual

COMPENSATION: The listed compensation range listed is paid bi-weekly per our standard payroll practices. Final base pay decisions are dependent upon a variety of factors which may include, but are not limited to: skill set, years of relevant experience, education, location, and licensure/certifications.   • Guidehealth is a fully remote company.
  • We provide new employees with the necessary equipment to function in their role at no charge to the employee.
