The LVN will assess patient needs, implement nursing care plans, and provide basic treatments such as wound care and catheterization. They are also responsible for administering medications and maintaining electronic health records for adults with IDD and autism.

This will be 32 base hours a month with the opportunity to work additional hours as needed. You will work as part of a healthcare team to assess patient needs, plan and modify care, implement individual nursing care plan. You will provide basic patient care or treatments such as dressing wounds, treating bedsores, massaging, or performing catheterizations and measure and record patient’s vital signs, administer medications and maintain information in electronic health system. Must be a graduate of a Licensed Vocational Nursing Program and have a valid License to practice as LVN in California (or ability to obtain/transfer). Prior experience preferred.
