Responsibilities include reviewing emails, checking phone messages, following up on referrals, reviewing documentation, ensuring proper paperwork and consents are completed and signed, conducting discharge planning, conducting utilization review, working well with other case managers, demonstrating strong communication skills, and working with a team.
