Make a Real Impact – Help patients restore function and regain independence One-on-One Care – Build meaningful relationships in a home setting Autonomy with Support – Manage your day with guidance from an experienced PT team Collaborative Environment – Partner with therapists, nurses, and care teams Growth & Development – Ongoing training and career advancement opportunities Work-Life Balance – Flexibility designed for field-based care Comprehensive Benefits – Medical, Dental, Vision, 401(k) match, PTO, and more What You’ll Do Deliver High-Quality Patient Care Provide direct physical therapy services under the direction of a licensed Physical Therapist Follow individualized plans of care to help patients reduce pain, improve mobility, and restore function Assist patients with therapeutic exercises, gait training, balance, and functional mobility Support patients with transfers, strengthening, and activities of daily living Utilize therapeutic modalities such as heat, cold, ultrasound, and electrical stimulation as appropriate Educate patients and caregivers on exercises, safety, and use of assistive devices Monitor Progress & Communicate Observe and report patient progress, response to treatment, and any changes in condition Maintain accurate, timely documentation in the EMR Communicate effectively with the Physical Therapist and interdisciplinary care team Participate in case conferences and care coordination efforts Collaborate & Contribute Work closely with nurses, therapists, and other healthcare professionals to ensure coordinated care Participate in team meetings, in-services, and ongoing training Uphold clinical standards and contribute to quality improvement efforts What Sets You Apart Strong patient-first mindset with a passion for rehabilitation Ability to motivate and engage patients throughout recovery Excellent communication and time management skills A compassionate, proactive approach aligned with our values: Trustworthy.

What You Bring Graduate of an accredited Physical Therapy Assistant (PTA) program Current PTA license or certification as required by state law 1+ year of experience preferred (home health experience a plus) Current CPR certification Valid driver’s license, reliable transportation, and auto insurance

Requirements

Must be a graduate of an accredited PTA program with a current state license and CPR certification. A valid driver's license and reliable transportation are required, with one year of experience preferred.
