Provide person-centered physical therapy care to residents to improve mobility, strength, and overall quality of life. Responsibilities include conducting assessments, developing individualized treatment plans, and collaborating with an interdisciplinary team.

Join our team as a full-Time physical therapist (Pt)

We are seeking a dedicated and compassionate Physical Therapist (PT) to join our in-house team of outstanding professionals. In this role, you will provide person-centered care to help residents achieve positive outcomes and improve their mobility, strength, and overall quality of life.

  • Your impact as a physical therapist (Pt)
  • Consult with patients to assess their physical condition and functional limitations
  • Evaluate and interpret patient assessments and test results
  • Develop individualized treatment plans using a variety of therapy techniques
  • Create personalized rehabilitation programs tailored to each patient’s goals
  • Administer medically prescribed physical therapy treatments to relieve pain, restore function, and promote healing
  • Ensure all treatments are delivered in accordance with the established plan of care
  • Monitor patient response to treatment interventions and modify approaches as clinically appropriate in collaboration with the interdisciplinary team
  • Provide clinical support and education to patients, family members, and caregivers
  • Collaborate with interdisciplinary team members to ensure high-quality patient care
  • Complete documentation thoroughly and timely in compliance with company policies and state/federal regulations
  • Comply with all applicable rules, regulations, and procedures
  • Attend required meetings as designated by the Director of Rehab

  • What makes you a great fit
  • Active and valid Texas Physical Therapist license
  • In good standing with all regulatory agencies and licensing boards
  • Skilled Nursing Facility (SNF) or long-term care experience preferred
  • Current knowledge of physical therapy treatment practices
  • Experience with Net Health Electronic Documentation System preferred
  • Ability to manage patients with varying personalities and needs
  • Strong communication, collaboration, and clinical decision-making skills

Requirements

Requires an active and valid Texas Physical Therapist license and good standing with regulatory boards. Experience in Skilled Nursing Facilities (SNF) and proficiency with Net Health documentation are preferred.
