About the role

Provide direct assessment, diagnosis, and holistic treatment plans for patients with acute and chronic wounds, including venous and lymphatic diseases. Coordinate care with multi-disciplinary teams and manage diagnostic imaging results to ensure effective wound healing.

Job Summary

Provide high-quality health care through active participation in care for patients in Wound Ostomy Department via direct assessment and formulation/implementation of treatment plan. Performs complete work-up on patients, including H & P, ordering of diagnostic tests, coordination of care with multi-disciplinary healthcare team, and patient teaching. Document in writing all care in patients permanent medical record in accordance with accepted medical/legal standards. Provides all medical care in accordance with accepted practice standards, hospital/clinic protocols (including supervision by a physician), and State licensing guidelines.

Essential Responsibilities

  • Provide direct assessment, diagnosis and formulation/ implementation of holistic treatment plan for patients presenting with acute and/or chronic wounds in the ambulatory care setting. Responsibilities include histories and physicals, exams, ordering diagnostic studies and medications, screening referrals, and wound management. Significant time is spent handling patient inquiries regarding care plan and coordination of care with members of multi-disciplinary healthcare team.
  • Venous and lymphatic disease: provide direct assessment, diagnosis, and formulation/ implementation of treatment plan for patients presenting with venous stasis and/or lymphatic disease. Responsibilities include histories and physicals, ordering diagnostic studies and medications, application of the principles of compression therapy, screening referrals. Significant time is spent handling patient inquiries regarding plan of care.
  • Neuropathic ulcers including
  • Diabetic
  • Foot
  • Ulcers (DFUs): provide direct assessment, diagnosis, and formulation/ implementation of treatment plan for patients presenting with neuropathic ulcers. Outpatient responsibilities include histories and physicals, ordering diagnostic studies and medications, screening referrals. Significant time is spent handling patient inquiries regarding care plan and coordination of care with multi-disciplinary healthcare team. Review and manage imaging results including X-rays, US, CT scans, MRA, angiograms as relates to wound healing and in coordination with multi-disciplinary healthcare team. Duties include ordering future appropriate diagnostic studies and care management of wounds. Significant time is spent with care coordination with patient and healthcare team.

Basic Qualifications

  • Experience Minimum one (1) year of experience medically managing a wide variety of complex acute and chronic wounds in a clinic or skilled setting. Minimum one (1) year of thorough and working knowledge of complex acute and chronic wound pathophysiology and their evaluation, management and treatment Minimum one (1) year of thorough and working knowledge of peripheral vascular diseases, diabetes, diabetic foot wounds, and general underlying pathologies that affect wound healing with an emphasis in limb preservation Education Masters degree as PA, NP, or closely related field. Graduate of ANA-approved nurse practitioner training program Bachelors degree as PA, NP, or closely related field. License, Certification, Registration This job requires credentials from multiple states. Credentials from the primary work state are required at hire. Additional Credentials from the secondary work state(s) are required post hire. Nurse Practitioner License (Oregon) within 6 months of hire AND Nurse Practitioner National Certificate required at hire AND Registered Nurse License (Oregon) within 6 months of hire AND Nurse Practitioner License (Washington) within 6 months of hire AND Registered Nurse License (Washington) within 6 months of hire OR Physician Assistant License (Washington) within 6 months of hire AND Physician Assistant National Certificate required at hire AND Physician Assistant License (Oregon) within 6 months of hire Basic Life Support National Provider Identifier required at hire

  • Requirements: Working knowledge of state health care laws, licensing board regulations and controlled substance regulations. Ability to know limitations and when to refer patients to specialty care teams. Ability to assess, diagnose and plan effective treatment for both acute and chronic wounds. Ability to see large volume of patients on a daily basis, in ambulatory settings. Ability to provide virtual support to other clinics and care teams. Additional training in wound care

Preferred Qualifications

Greater than two (2) years thorough and working knowledge of wound care and wound assessment and management, especially in diabetic feet, of chronic wounds in an ambulatory care setting.

Notes: Working schedule = 4 8-hr shifts M-F with occasional weekends and holidays

Requirements

Requires a Master's degree as a Nurse Practitioner or Physician Assistant with at least one year of experience in complex wound management. Must hold or obtain valid licenses and certifications for both Oregon and Washington states.
