SUMMARY: The Community Support Health and Wellness Specialist’s position works collaboratively within an interdisciplinary treatment team to provide health and wellness case management services to adults with mental illness and medical challenges as defined by DC Mental Health Rehabilitation Services regulations using a Strengths Based Recovery Model. The position performs administrative work providing accurate and comprehensive documentation of all services within specified time limits.
EDUCATION and EXPERIENCE: GED or HS Diploma. Medical Assistant Certification preferred. 2 years of direct service experience in a social service setting with adults who have mental illness. SKILLS and COMPETENCIES: Driving is required. Must have valid local driver’s license and clean driving record. Work requires the ability to independently travel to various service locations and to transport consumers. At least 50% of the time is spent in the community providing services to consumers in their natural settings. Skill in the use of computer systems (preferably in a PC, Windows-based operating environment) and MS Office products. Ability to learn Catholic Charities’ electronic health record (Credible).
