The Social Worker is responsible for patient assessments, care planning, and providing counseling and psychosocial support. They collaborate with an interdisciplinary team to identify interventions and rehabilitation goals to improve patient quality of life.
Responsibilities: include Patient Assessment/Care Planning and Counseling, Psychosocial Assessment;
KDQOL, Clinical Depression Screen, Referrals to outside agencies, Transportation needs; Health Insurance, Medication coverage, and Collaboration with billing. Proficiency in written communication is required to complete monthly social work notes, Depression Screening, Quarterly MSPQ’s, and other required documentation. Social Worker also Participates as a Member of the Interdisciplinary Team to help identify effective interventions and rehabilitation goals to improve patients’ quality of life.
Candidates must be a Licensed Social Worker with some professional experience. Proficiency in written communication is required for completing monthly notes and mandatory clinical documentation.
