Physicians Choice Home Health is hiring a full-time or part-time/per diem Occupational Therapist to join our Home Healthcare Team and see patients in Southern California's South Bay Area. In this role, you will provide high-quality occupational therapy services to patients in the comfort of their own homes, helping them regain independence and improve their daily functioning. You will supervise a Certified Occupational Therapy Assistant (COTA) and work with a team of dedicated healthcare professionals who are committed to delivering exceptional patient-centered care. What You'll Do Provide in-home occupational therapy evaluations and treatments according to the physician’s plan of care Assess patients’ strength, coordination, mobility, and ability to function safely at home Develop and update individualized treatment plans based on patient goals and progress Help patients improve independence with daily activities such as dressing, bathing, mobility, and home safety Educate patients and caregivers on exercises, adaptive techniques, and safe movement Document evaluations, visits, and progress in WellSky/Kinnser in a timely manner Communicate patient progress, concerns, and changes in condition to the care team and physician Supervise COTAs in accordance with state guidelines Participate in care planning, case conferences, and discharge planning Graduate of an accredited Occupational Therapy program (Master’s or Doctorate in Occupational Therapy) Active California Occupational Therapist license Current CPR/BLS certification Reliable transportation and willingness to travel to patient homes Home Healthcare experience Experience documenting in the Kinnser/Wellsky EMR preferred, but not required $40-120/hour, depending on task/visit type Mileage reimbursement Cellular device reimbursement Eligibility for Obran Cooperative membership and profit-sharing Full-time packages also include Medical, dental, vision, life & disability insurance, with generous employer contributions HSA/FSA options to cover healthcare expenses and dependent care with pre-tax dollars 401(k) Paid time off, sick time, and paid holidays

Salary Range

Requirements

Must be a graduate of an accredited Occupational Therapy program with an active California license and CPR/BLS certification. Reliable transportation is required for travel to patient homes in the South Bay Area.
