About the role

Providers will conduct in-home visits, assess living environments, address safety concerns, and coordinate care transitions to reduce hospitalizations and support aging in place. Responsibilities include chronic disease management, medication reconciliation, preventive care, palliative support, and care plan oversight.

Introduction to the Role and Practice Centra’s PACE (Program of All-Inclusive Care for the Elderly) is a nationally recognized model of care serving adults aged 55 and older in our community.

The mission of PACE is to deliver person-centered care in the setting that’s most appropriate for each participant—most often in the comfort and safety of their own home.

Our home-based care approach allows participants to maintain independence while receiving comprehensive medical, social, and supportive services.

PACE is a capitated model of care, which means there are no productivity metrics, only a focus on delivering the care each participant truly needs.

In this home health–focused model, providers conduct in-home visits, assess living environments, address safety concerns, and coordinate care transitions to reduce hospitalizations and support aging in place.

Everything is coordinated with a dedicated interdisciplinary team of providers, nurses, dietitians, physical and occupational therapists, social workers, home health aides, activities coordinators, and transport staff to ensure seamless, wraparound care.

Centra’s PACE program is seeking a dedicated Advanced Practice Provider to join our team and help shape the future of our home-based care model.

This is a unique opportunity to deliver comprehensive, relationship-centered care directly in participants’ homes and community settings.

You will play a vital role in chronic disease management, medication reconciliation, preventive care, palliative support, and care plan oversight—meeting patients where they are physically and clinically.

Requirements

Candidates must be graduates of an accredited Nursing Program with a Master of Science Degree in Nursing (MSN). Required licensure includes being a Nurse Practitioner with prescriptive authority in Virginia, maintaining BLS and ACLS Certifications, and holding a current, active RN license in Virginia.
