Provide outpatient primary care services at San Tan Valley and Baseline clinics, managing an average of 18-22 patients per day. Collaborate with a dedicated MA team and utilize EPIC EMR to improve community health outcomes.

  • Position Highlights
  • Practice outpatient Primary Care at the full scope of your license
  • Full-time position, Monday through Friday
  • Provide care at our San Tan Valley and Baseline Clinics
  • 36 patient contact hours each week
  • 18-22 patients per day on average
  • EPIC EMR system with Dragon dictation and AI notetaking
  • Dedicated MA team for rooming and support
  • Onsite lab and pharmacy resource support for medication refills
  • Out-of-state applicants encouraged to apply, relocation assistance available

Required Qualifications

  • Unrestricted Arizona Nurse Practitioner or Physician Assistant license or ability to obtain by start date
  • National Nurse Practitioner certification through AANC or AANP, or Physician Assistant national certification through NCCPA
  • Minimum two (2) years of experience as a primary care provider
  • Current DEA registration or ability to by start date
  • Current BLS certification or ability to by start date

Preferred Qualifications

  • Experience in providing patient care under a Value Based Care Model
  • Experience in outpatient Primary Care
  • Knowledge of HEDIS measures, CAHPS, HOS, HCC Coding documentation
  • Proficient in Medicare Annual Wellness visits
  • Proficient in EMR systems & technology (EPIC Preferred)
  • The salary range for this role is $104,500 to $156,000 per year.
  • Salary Range is defined as total cash compensation at target.
  • The actual range and pay mix of base and bonus is variable based upon experience and metric achievement.
  • Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc.

Requirements

Requires an unrestricted Arizona NP or PA license, national certification (AANC, AANP, or NCCPA), and at least two years of primary care experience. Must possess current DEA registration and BLS certification.
