We're hiring compassionate Home Health Medical Social Workers (MSWs) in Oakland, Macomb and Washtenaw counties to provide personalized, one-on-one support that helps patients and families navigate life's challenges, access essential resources, and remain safe and independent at home.

Deliver skilled medical social work services in accordance with physician orders and the CMS Conditions of Participation (CoPs).
Maintain timely, accurate documentation while ensuring compliance with CMS, payer, and agency standards.
Collaborate with patients, caregivers, physicians, and the interdisciplinary team to promote safe, effective care and successful discharge planning.
Provide counseling, emotional support, caregiver education, crisis intervention, and referrals to community resources, financial assistance, transportation, Durable Medical Equipment, long-term care, and other support services.
Advocate for patients, address psychosocial barriers to care, and communicate changes in patient status to the care team. 💼 What We're Looking For Master’s degree in social work (MSW) from an accredited program.
Current Michigan Social Work license (LMSW or LLMSW) in good standing.
Home health or medical social work experience preferred.
Strong assessment, critical thinking, crisis intervention, and problem-solving skills.
Excellent communication, counseling, and interpersonal skills.
Ability to work independently while collaborating effectively with an interdisciplinary team.

A Master’s degree in social work and a current Michigan Social Work license are required. Home health or medical social work experience is preferred, along with strong assessment and communication skills.
