Job Description

Works in coordination with interdisciplinary teams to organize, prepare, and deliver evidence based clinical services across the continuum of care. Maintains ethical principles and professional standards of practice in adherence to the discipline’s professional code of ethics.

Performs clinical diagnosis in conjunction with conducting a comprehensive bio-psychosocial assessment and establishment of a care plan.

Champions integrated care model treatment guidelines in the primary care setting by providing approximately 6-8 sessions of short-term, focused treatment.

Qualifications

Minimum Requirements Education Master’s Degree in Social Work, or Counseling, or Marriage and Family Therapy or Related Human Service Field Experience Previous experience in a healthcare setting 3-5 years’ experience as a licensed clinician is preferred Licensure, Certification, Registration LCSW, LPC, LMFT, LADC Valid Driver's License We take great care of careers.

Requirements

Candidates must hold a Master's degree in Social Work, Counseling, Marriage and Family Therapy, or a related human service field. Required licensure includes LCSW, LPC, LMFT, or LADC, along with a valid driver's license.
