Some highlights of this opportunity include: · Participate in the decision-making process in relation to admission to hospice at home or in the inpatient setting · Participating as a member of the Interdisciplinary Team in the development of individualized care plans · Certify (re-certify when necessary) patients eligible for hospice care under the Medicare Hospice Benefit · Consult with attending physicians regarding pain and symptom management · Coordinate efforts with the attending physician to provide continuity of care · Develop and monitor the medical component of the hospice program · Participate in the development, coordination and implementation of hospice services and policies · Participating in the development of staff and volunteer training · Conduct home visits (in person and virtual) as a consulting physician when appropriate · Act as a hospice liaison and advocate with the national, state and local physicians, healthcare professionals, partners and community · Perform related administrative functions as required and/or assigned ·

Schedule:

Forty-eight (48) on call weeks per year: Monday 7am through Friday 7pm. Including mandatory in person attendance of 1 Inter Disciplinary Team Meeting (IDT) per week.

Four (4) weekends per year: Friday 7pm through Monday 7am
