We're hiring compassionate Home Health Physical Therapists (PTs) and Physical Therapist Assistants (PTAs) in Macomb, Wayne and Washtenaw counties to provide personalized, one-on-one care that helps patients regain strength, improve mobility, reduce pain, and safely return to the activities they love.

If you're looking for a flexible schedule, meaningful patient relationships, and the opportunity to make a lasting impact every day, we'd love to meet you. 💙 You Might Be a Great Fit If You: Conduct comprehensive physical therapy evaluations to assess functional needs, develop individualized treatment plans, provide evidence-based interventions, and monitor progress to maximize mobility, safety, and independence.

Deliver skilled physical therapy services in accordance with physician orders and the CMS Conditions of Participation (CoPs).

Maintain timely, accurate documentation while ensuring compliance with CMS, payer, and agency standards.

Collaborate with patients, caregivers, physicians, and the interdisciplinary team to optimize mobility, functional outcomes, and fall prevention.

Educate patients and caregivers on therapeutic exercises, gait training, transfer techniques, fall prevention, assistive devices, and individualized Home Exercise Programs (HEPs) to promote safe mobility and recovery.

Reassess patient progress, modify treatment plans as appropriate, and communicate changes to the care team. 💼 What We're Looking For: Degree from an accredited Physical Therapy (PT) or Physical Therapist Assistant (PTA) program.

Current Michigan Physical Therapist/Physical Therapist Assistant license in good standing.

Home health experience preferred (minimum one year).

Strong clinical judgment, critical thinking, and problem-solving skills.

Excellent communication, interpersonal, and patient education skills.

Ability to work independently while collaborating effectively with an interdisciplinary team.
