Advanced Practice Provider (NP/PA/APP) - UVA Health Mobile Care

UVA Health’s Mobile Care program is bringing high-quality primary care directly into the community, expanding access for patients who need it most while redefining how care is delivered across the region.

This is a unique opportunity for an experienced Advanced Practice Provider to combine independent clinical practice with meaningful program development.

You will practice at the top of your license in a highly autonomous environment while helping build and shape a growing population health initiative.

The Role: This position blends clinical excellence, adaptability, and leadership within an innovative care delivery model.

As the first full-time APP dedicated to Mobile Care, you will play a critical role in both patient care and the continued growth of this emerging program.

Working within a mobile unit that travels to community sites across a 40-mile radius, your day-to-day experience will be dynamic, patient-focused, and highly impactful.

This role is ideal for an experienced NP or PA who thrives in independent practice, values flexibility, and is energized by the opportunity to deliver care in innovative ways.

Due to the highly independent nature of this position, candidates with an Autonomous Licensure are highly preferred.

In this role, you will: Provide comprehensive primary and preventative care in community-based and non-traditional settings Deliver care through a mobile, community outreach model, improving access for patients with transportation or access barriers Practice with a high level of clinical autonomy, making independent decisions in real-time Collaborate closely with a dedicated mobile care team consisting of a nurse and paramedic Engage with a diverse patient population, addressing a wide spectrum of needs Support the growth and development of the program, including building workflows and improving operational processes Adapt to changing locations and environments, creating a highly varied and engaging clinical experience Advanced Practice Provider Scope of Practice: Clinical Provides expert evidence-based, top-of-scope care in their clinical practice Acts as part of a specialty team, collaborating with other clinicians as appropriate Provides health education to patients Maintains accurate, timely, and thorough documentation Ensures compliance with institutional and regulatory standards Leadership Supports development of best practices and protocols in specialty Serves on committees and workgroups to enhance patient care Provides education to learners in the clinical space Mentors new team members Administration Contributes to clinical quality improvement work Participates in professional governance through the APP Council Attends team development meetings at local and departmental levels Professional Adheres to ASPIRE values in all professional interactions Maintains appropriate licenses, certifications, and specialty training competencies Participates in local, state, and national professional organizations https://app.uvahs.org/ Website links: https://www.uvahealth.com/support/mobile-care https://www.medicalcenter.virginia.edu/population-health/programs/mobile-care1/ News Clips: https://www.29news.com/2025/10/14/uva-health-launches-mobile-clinic-reach-rural-virginia-patients/ https://www.youtube.com/watch?v=FFQJKBKMd7I Background checks and pre-employment health screenings will be conducted on all new hires prior to employment.

MINIMUM REQUIREMENTS Education: Graduate of an accredited graduate school of nursing or physician assistant program with a master’s degree.
