Under the supervision of a Care Manager, the Community Health Worker provides essential administrative, logistical, and direct-support assistance to facilitate the delivery of care management services to CALAIM authorized members. The CHW supports the care management team in coordinating community-based services, conducting outreach, processing referrals, and ensuring clients receive timely and dignified access to the programs and resources they need. This role requires compassion, attention to detail, strong organizational skills, and the ability to work effectively with a diverse population.
• High school diploma or GED, prior experience in community social services, health education and care coordination.
$58,000 - $70,000 annually Full-Time, Monday to Friday, In-Person, and 37.5 hours week
This position is full-time, and the compensation is commensurate with the selected candidate's qualifications and experience.
