Evolutionary Healthcare is seeking compassionate Nurse Practitioners (NPs) and Physician Assistants (PAs) to join our innovative Osteopathic Medicine team serving skilled nursing and post-acute care facilities throughout Central Florida.
This unique role is ideal for providers who enjoy hands-on patient care, musculoskeletal medicine, pain management, and working closely with Osteopathic Physicians to improve mobility, function, and quality of life. While OMT is performed by our physicians, you will play a vital role in identifying appropriate patients, managing their medical care, and supporting comprehensive treatment plans.

Enjoy working with geriatric and post-acute patients Have an interest in musculoskeletal medicine and functional improvement Value teamwork and physician collaboration Are passionate about improving mobility, comfort, and independence Thrive in relationship-based patient care
Licensed Nurse Practitioner or Physician Assistant in Florida (or ability to obtain) National Board Certification DEA license (or ability to obtain) Experience in Internal Medicine, Family Medicine, Geriatrics, Hospital Medicine, or Post-Acute Care preferred Skilled Nursing experience is a plus but not required Strong communication and clinical decision-making skills Passion for patient-centered care Why This Role Is Different Unlike traditional primary care positions, this role allows you to become part of an integrated Osteopathic Medicine program focused on restoring function—not simply treating disease.
Working alongside experienced Osteopathic Physicians, you'll help identify patients who can benefit from OMT, support individualized care plans, and contribute to improving mobility, reducing pain, preventing falls, and enhancing quality of life for residents in skilled nursing facilities.

Must be a licensed Nurse Practitioner or Physician Assistant in Florida with National Board Certification and a DEA license. Experience in Internal Medicine, Family Medicine, Geriatrics, or Post-Acute Care is preferred.
