The LPN is responsible for monitoring patients’ health, administering basic nursing care, and ensuring that patients’ medical needs are met. This role is essential in promoting the health, comfort, and well-being of residents by delivering high-quality nursing care in a compassionate and professional manner.

  • Care: Provide basic nursing care to residents, including administering medications, wound care, and assisting with personal hygiene. Monitor residents' vital signs, such as temperature, blood pressure, and pulse, and report any abnormalities to the supervising nurse or physician. Assist with feeding, dressing, and mobility, ensuring residents’ comfort and safety. Follow individualized care plans and provide care according to residents’ needs and medical conditions.

  • Administration: Administer medications to residents as prescribed by physicians, ensuring accurate documentation and timely delivery. Monitor residents for adverse reactions to medications and report any concerns to the supervising nurse or physician. Maintain accurate records of medication administration and residents’ responses to treatments.

  • Reporting: Maintain up-to-date and accurate documentation of patient care, including vital signs, treatments, and any changes in condition. Report residents’ progress, concerns, or changes in health status to the supervising nurse or physician. Ensure that all documentation complies with facility policies and healthcare regulations.

  • Communication: Work closely with RNs, physicians, and other healthcare professionals to ensure cohesive patient care and effective communication. Communicate with residents and their families, addressing concerns and providing updates on residents' health and treatment. Participate in care conferences and interdisciplinary team meetings to discuss resident care plans and outcomes.

Licensure

Current and active Licensed Practical Nurse (LPN) license.

Experience

Previous experience in a healthcare or long-term care setting is preferred but not required. New graduates are welcome to apply.

Requirements

Candidates must have completed an accredited LPN or LVN program and hold a current, active LPN license. While previous long-term care experience is preferred, new graduates are welcome to apply.
