  • Responsibilities: Perform thorough evaluations, assessments, and treatments for patients recovering from illness, injury, or surgery Develop and implement individualized care plans and therapeutic interventions to improve patient mobility and function Collaborate closely with physicians, nurses, and other rehabilitation team members Educate patients and families on therapy plans, mobility techniques, and home exercise programs Accurately complete all required documentation and patient records in a timely manner Ensure care and services meet regulatory, facility, and professional standards Participate in quality improvement initiatives and staff in-services

LICENSURE, CERTIFICATION AND EDUCATIONAL REQUIREMENTS Active, current, unrestricted Physical Therapist (PT) license in the state of practice Graduate of an accredited Physical Therapy program (CAPTE accredited) Experience in skilled nursing, rehabilitation, or post-acute care preferred Strong clinical, communication, and organizational skills Commitment to patient-centered, compassionate care Family Makes Us Stronger.

Requirements
