About the role

Acts as the MIHP Care Coordinator by performing home and office visits to conduct medical and nutritional assessments. Develops team care plans and manages referrals to ensure comprehensive client services.

Essential job duties

  • Functions as the MIHP Care Coordinator, as assigned. (40%) a.
  • Performs home and office visits, completing psychological/sociological/medical/ and nutritional assessments according to protocol and providing counseling as deemed necessary based on the assessment. b.
  • Participates in team care plan development and monitoring through case conferences with all Disciplines working with the client, to include other agencies involved with the client. c.
  • Makes referrals, both within and outside of GLBHC programs for needed services. d.
  • Provides or arranges for follow up with other team members.
  • 2.
  • Completes and submits all required documentation for client record in compliance with MIHP standards. (30%) 3.
  • Complies with other billing and record keeping requirements of the program. (20%) 4.
  • Participates in quality improvement activities. (10%)

Job specifications

1.

Education

Associates Degree in Nursing required; Bachelor’s or Master’s degree preferred. Possession of an M.S.N. or B.S.N. and at least one year of providing community health, pediatrics and/or maternal/infant nursing services; or (2) possession of a diploma or A.D.N. and at least two years of providing community health, pediatric and/or maternal/infant nursing services. All degree nurses must be in possession of a current Michigan license to practice nursing in Michigan under Part 172 of Michigan Public Act 368 of 1968, as amended. 2.

Licensure

RN, BSN or MSN Must have a current Michigan license to practice under Part 172 of Michigan Public Act 368 of 1968, as amended. CPR certification required. 3.

Experience

MSN or BSN with at least one year of providing community health pediatric and/or maternal/infant nursing services OR ADN with at least two years of providing community health, pediatric, and/or maternal/infant nursing services. Previous fieldwork in the service area preferred. 4.

Skills

  • Able to communicate effectively with a diverse population. Ability to coordinate patient services and deal effectively with other service organization personnel. 5.
  • Interpersonal
  • Skills: Organized and effective with limited direct supervision. Ability to provide services in an empathetic, nonjudgmental and objective manner. Comfortable visiting clients in their homes, and in deteriorated(-ing) and high-risk neighborhoods. Flexible in scheduling and accepting referrals from other members of the care team. 6.
  • Physical
  • Effort: Must be able to sit, stand, and or walk for an entire workday. Must be able to lift, carry, push, pull, and or twist while holding up to 25 lbs. frequently. 7.

  • Work: 40 hours per week; flexible and varied. 8.

Travel

Extensive travel is required within the service area involving use of company or personal vehicle with reimbursement for mileage provided at the existing rate of the corporation. Minimal out-of-service area travel may be required in order to attend conferences/training. Reimbursement consistent with GLBHC policy. Must be a licensed driver with proof of personal liability and property damage insurance.

Requirements

Requires an Associate Degree in Nursing (ADN) with two years of experience, or a BSN/MSN with one year of experience in community, pediatric, or maternal health. Must hold a current Michigan RN license and CPR certification.
