As a Licensed Practical Nurse (LPN) or Graduate Practical Nurse (GPN) on our Chronic Care team, you'll provide compassionate, patient-centered care to individuals with complex, long-term medical needs. Working under the supervision of a Registered Nurse, you'll collaborate with the interdisciplinary team to assess patient needs, implement individualized care plans, monitor patient progress, and support positive clinical outcomes. Must be a graduate of an accredited LPN program with current licensure in the State of Connecticut. CPR certification is required within three months, and one year of medical/surgical or long-term care experience is preferred.
