The Clinical Pharmacist optimizes patient medication therapies by monitoring dosages, interactions, and allergies while performing medication reconciliation. They serve as a drug information resource for medical staff and collaborate with Intensivists and Hospitalists during patient rounds.
7 on 7 off position with hours 10: 30am-9pm
Decentralized unit-based Clinical Pharmacists are valued team members working in patient care areas optimizing patient medication therapies by monitoring drug selection, drug dosage, frequency, food/drug allergies, drug- drug interactions and duplication, medication reconciliation and discharge medications. They are a resource for drug information and education for physicians, nurses, and patients. Also, dose medications under protocols. They also round with Intensivists and Hospitalists playing an active role making recommendations for therapies and changes in therapies.

Requires a Doctorate of Pharmacy or a Bachelor's Degree in Pharmacy with 5 years of clinical experience and a South Carolina license. BLS and ACLS certifications are required, while residency training and board certification are preferred.
