About the role

Provide personalized occupational therapy services to adult and geriatric patients to improve functional independence and quality of life. Implement treatment plans focused on activities of daily living (ADLs) and collaborate with interdisciplinary teams to ensure continuity of care.

Responsibilities

Evaluate patients and provide personalized speech therapy services that improve communication, cognition, swallowing function and overall quality of life Develop and implement patient-centered treatment plans focused on speech, language, cognitive and swallowing rehabilitation goals Deliver evidence-based therapy interventions that support functional independence in adult and geriatric patients Partner with interdisciplinary teams, caregivers, and nursing staff to support strong patient outcomes and continuity of care Maintain accurate clinical documentation, monitor patient progress and adjust treatment plans based on individual rehabilitation needs

Qualifications
