Job Summary and Responsibilities We are seeking a skilled and compassionate Physical Therapist to join the VNA Health at Home team.
The successful candidate will provide physical therapy services to patients in their homes, promoting rehabilitation, recovery, and improved quality of life.
As a key member of our interdisciplinary team, you will work closely with patients, families, caregivers, and other healthcare professionals to develop and implement personalized treatment plans that address each patient's unique needs and goals.

Practicing in the home setting, where patients spend most of their time, is the most “functional” place to make an impact on this!
Requires a bachelor's degree from an APTA-approved school, an active state PT license, and CPR/BLS certification. A valid driver's license is required, and one year of clinical experience is preferred.
