In this role, you will be responsible for managing the coordination of appointments, transportation arrangements, and facilitating effective communication between nurses and other team members. Additionally, you will ensure follow-through on medication orders and bring necessary information to appointments, across our Bristol YAS programs. What You Will Do Track, schedule and accompany people served on medical appointments Communicate relevant information to/from the medical providers and the rest of the IDT team Ensure all necessary clinical documentation is prepared and/or obtained as required, including health visits, medical records, etc. Ensure proper filing of documentation per agency processes Coordinating and/or attending with medical providers/offices Assisting with assessments, procedures and taking vital signs What You Will Need HSD or GED equivalent Minimum of 1 year of experience in a related field (CNA, or related college degree/studies preferred) DDS Medical certification required, any med training a plus! PPEC required or ability to obtain Ability to work independently and prioritize tasks effectively. Strong organizational skills and attention to detail. Proficiency in computer skills for documentation and communication purposes. Ability to problem-solve and take decisive action in real-time situations.
