Why This Role Matters As a Home Health Physical Therapist, you are a critical part of the care team - helping patients safely recover, regain mobility, and maintain independence where they feel most comfortable: at home. This is a highly autonomous, relationship-driven role where you can truly see the impact of your work every day. What You’ll Do Deliver Patient-Centered Care Evaluate and assess patients’ functional status, mobility, strength, and rehabilitation potential Develop and implement individualized care plans in collaboration with physicians and the care team Provide hands-on therapy to improve mobility, strength, balance, and independence Educate patients and caregivers on treatment plans, exercises, and safety strategies Drive Outcomes & Recovery Monitor patient progress and adjust treatment plans as needed Support safe transitions of care and contribute to discharge planning Identify additional patient needs and collaborate with interdisciplinary teams Collaborate & Communicate Partner closely with nurses, therapists, and physicians to ensure coordinated care Document patient care and progress accurately and timely within the EMR Participate in case conferences and team meetings Lead & Support Supervise Physical Therapist Assistants (PTAs) in accordance with state guidelines Provide guidance and education to caregivers and support staff What You Bring Graduate of an accredited Physical Therapy program (DPT or Master’s) Current PT license in the state of practice Minimum 1–2 years of experience (home health experience preferred) Strong clinical assessment and critical thinking skills Excellent communication and patient education abilities Current CPR certification Valid driver’s license, reliable transportation, and auto insurance What Sets You Apart You value autonomy and thrive in a field-based environment You’re passionate about patient independence and functional outcomes You build strong, trusting relationships with patients and families You bring both clinical excellence and compassionate care to every visit

Requirements

Requires a graduate degree in Physical Therapy (DPT or Master's) and a current state PT license. Candidates should have 1-2 years of experience, CPR certification, and a valid driver's license with reliable transportation.
