About the role

Triage and assess individuals and families facing economic insecurity to develop individualized comprehensive plans of care. Act as a facilitator and advocate to connect clients with community resources and navigate barriers to service implementation.

JSSA’s Mental Health Services (MHS) department has an immediate opening for a full-time Community Care Navigator to join our growing team in Fairfax, VA addressing economic insecurity. This position will report directly to the Community Care Navigation Team Leader and will be part of the Behavioral Health Support Services Team within MHS. We are a dedicated team of behavioral health professionals committed to providing cutting-edge services, programs, and support for children, adults and families.

Responsibilities

Engage with referred individuals and families to triage and assess needs stemming from economic insecurity. Complete necessary engagement and assessment tools. Develop an individualized, comprehensive plan of care clearly outlining the goals to be met utilizing community resources and creative interventions. Provide in-depth care navigation within a Systems of Care framework to support individuals and families in assigned region(s) (DC, MD or VA) who require substantive assistance and navigation of resources to meet the needs related to economic insecurity. Serve as a facilitator, advocate, liaison, and partner working with individuals/families to connect to needed supports and services ensuring interventions meet needs. Identify services and resources to support the successful implementation of interventions. Identify and address gaps in services and supports which prevent the successful implementation of interventions and support individual/family with navigating barriers until resolution. Develop understanding of services and supports in the region and establish working partnerships where appropriate to be utilized in the continuum of support as needed. Position Qualifications: Bachelor’s degree in social work, psychology, or a related field required. Minimum of 2-3 years of experience performing the duties outlined above in DC, MD or VA. Experience navigating social welfare systems. Must be proficient working in virtual environment and comfortable engaging supervisory and team support. Proficient computer/database skills (Microsoft Office), minimum of 2 years of experience working with an electronic health record or similar technology related to service delivery. Requirements

Requires a Bachelor's degree in social work, psychology, or a related field with 2-3 years of experience in care navigation. Must be proficient in Microsoft Office and have at least 2 years of experience using electronic health records.
