We are currently seeking a Licensed Behavioral Health Therapist (LCSW, LMFT, or LMHC) to join our team.

This position offers the chance to serve an established panel of patients in a medically underserved community while working alongside in-house wraparound support, including Outreach Specialists, Care Managers, and Patient Care Coordinators.

Job Summary As a Licensed Behavioral Health Therapist, you will provide psychosocial assessments, therapeutic counseling, and crisis intervention for patients and families.

You’ll collaborate with an interdisciplinary healthcare team to ensure continuity of care, support smooth transitions, and promote quality patient outcomes.

This role is ideal for a mission-driven therapist who thrives in a collaborative, community-based setting.

Job Responsibilities (include but are not limited to): Conduct comprehensive psychosocial assessments and develop individualized treatment plans Provide short-term therapeutic counseling and crisis intervention to patients and families Collaborate with physicians, nurse practitioners, and the broader care team to ensure integrated care Coordinate services and support smooth transitions across levels of care Maintain accurate and timely clinical documentation in the Electronic Health Record (EHR) Apply knowledge of protective service mandates for both adults and children Utilize de-escalation and crisis intervention techniques when needed Build and maintain strong relationships with community resources, medical teams, and protective services Participate in team-based care meetings and continuous improvement initiatives Required Skills and Qualifications Master’s Degree in Social Work, Counseling, or Marriage & Family Therapy Active Indiana licensure as LCSW, LMFT, or LMHC (required) CPI (Non-Violent Crisis Intervention) certification preferred Relevant experience in healthcare, mental health, or community-based services Strong knowledge of disease processes, human behavior, and principles of growth and development Ability to provide effective therapeutic counseling and crisis intervention Excellent interpersonal and communication skills; ability to collaborate in a team-based setting Why You’ll Love Working Here Purpose-driven work that directly impacts access to care across our communities Robust benefits package (medical, dental, vision) designed to support you and your family Generous PTO because we believe caring for others starts with caring for yourself 401(k) with employer contribution to help you plan for what’s ahead Life and disability coverage for peace of mind Here, you are not just filling a role—you are helping shape healthier communities and advancing equitable care every day!

Requirements

Requires a Master's degree in Social Work, Counseling, or Marriage & Family Therapy and active Indiana licensure as an LCSW, LMFT, or LMHC. Experience in healthcare or community-based mental health services is required, with CPI certification preferred.
