About the role

Conduct initial and ongoing comprehensive assessments of hospice patients and their families to determine care needs. Provide physical assessments and communicate health care instructions to patients and caregivers.

Provides a complete physical assessment and history of current and previous illness (es).

Communicates with others on the hospice team and provides health care instructions to patients and their caregivers.

QualificationsRN with current license, excellent observation, problem solving, verbal and written communication skills.

Must be dependable and self-directed and able to work with minimal supervision.Additional Information401K with matching opportunities, Dental, Vision and Medical Insurance.

Requirements
